Send the filled form to apcf@ust.hk (User must send request via HKUST email account) to initiate the request.
LAF staff responsible to your request will contact you for more details on your request and make arrangement with you accordingly.

Important Note: User requesting service must get permission from the budget controller prior to service requisition.
